Referral: Marcus, 20, was referred by probation following release from a short custodial sentence. He had no fixed address, a history of cannabis use, and no employment history. He was at high risk of reoffending and homelessness.
Intervention: Placed on the Champions Pathway Tier 2 (Housing Programme). Allocated a key worker and enrolled in structured boxing sessions three times per week. Weekly mentoring sessions focused on anger management, budgeting, and goal-setting. Supported to register with a GP and access substance misuse services.
Outcome: After 8 months, Marcus sustained his tenancy, completed a Level 1 fitness qualification, secured part-time employment at a local gym, and had not reoffended. He was supported to move on to independent accommodation with floating support.
Referral: Sarah, 31, was referred by an IDVA following domestic abuse. She arrived with two children, no income, no legal representation, and a non-molestation order in process. She had severe anxiety and was not registered with any local services.
Intervention: Placed in women-only accommodation with housing-related support. Key worker helped her register with a GP, apply for Universal Credit, access legal aid for her family court case, and enrol her children in a local school. Weekly wellbeing check-ins and safety planning throughout.
Outcome: After 9 months, Sarah had a secured non-molestation order, was receiving all entitled benefits, her children were settled in school, and she moved into a 2-bedroom housing association flat with continued floating support. She reported feeling safe for the first time in years.
Referral: David, 45, was referred by the NHS discharge team following a 3-month mental health inpatient stay. He had lost his private rented tenancy during admission and had no family support. Previous discharge attempts had failed due to lack of suitable step-down accommodation.
Intervention: Placed in Recovery Housing with intensive support from Angel Oak Care. Daily support with medication management, gradual reintroduction of independent living skills (cooking, cleaning, shopping), and liaison with the community mental health team. Supported to attend all outpatient appointments.
Outcome: After 10 weeks, David was stable, managing his own medication, and had been allocated a council flat. He moved on with a floating support package and has not been readmitted to hospital in the 6 months since.
Referral: Tyler, 14, was permanently excluded from school following persistent disruptive behaviour. He had been out of education for 5 months with no engagement in any provision. SEMH assessment identified anxiety, low self-esteem, and anger management difficulties. Previous PRU placement had broken down within two weeks.
Intervention: Referred to the Champions Engagement Hub by the local authority SEND team. Began with two boxing sessions per week plus weekly 1-to-1 mentoring. Timetable gradually increased to four sessions. Mentor worked on emotional literacy, conflict resolution, and building a daily routine. Family support included regular contact with Tyler’s mother.
Outcome: After 5 months, Tyler was attending every session, had not been involved in any incidents, and was assessed as ready to transition back into a PRU with part-time attendance. His school reported a significant improvement in his emotional regulation and willingness to engage with adults.
We don’t just collect data — we use it. Every service has a clear outcome framework that is monitored, reported on, and used to drive continuous improvement.
Every resident or participant has a personalised support plan with SMART goals. These are reviewed regularly with key workers, updated at least monthly, and shared with referrers where appropriate.
We track outcomes across key domains: housing stability, health & wellbeing, safety, education & employment, financial capability, and social inclusion. Progress is measured at entry, 3 months, 6 months, and exit.
We provide quarterly outcome reports to commissioners and referrers, including occupancy data, support hours delivered, move-on outcomes, safeguarding activity, and resident feedback. Bespoke reporting available on request.
